**NEW REGISTRATION PROCESS** – This year we begin the MOPH-LAMOPH Separate Registration process.  MOPH Members (Life & Associate) please use ONLY the MOPH Form; LAMOPH (Life & Associate) please use ONLY the LAMOPH Form – Each is to be accompanied with a single check for that form only. Because of the separate accounting, we will return combined checks.

 AIRPORT TRANSPORTATION:  After your luggage has been retrieved, find the courtesy phone located at the baggage claim area.  Dial 40 which is the number labeled “The Scottsdale Plaza Resort.”  Guests will be asked your name and your flight information – you will then be directed to the north curb “Resort Shuttles Vehicle Loading” area – distinguished by a sign with a blue diamond.  There is a $17.00 charge per person each way – includes the driver gratuity – this is posted directly to your room account.  Shuttle departs every half-hour on the hour and the half hour between 5:30 a.m. to 9:00 p.m. seven days a week.

 THURSDAY AFTERNOON EVENTS:  To book a tour, attendees should contact Scottsdale Plaza Resort Special Event Manager, Cynthia Kotfila, at cynthiak@scottsdaleplaza.com or 480-951-5144.  The cost of the tour will be billed to each guest’s room.
