Since you have met the criteria for joining the we look forward to welcoming you into Order.

There are three steps that must be completed before your membership is official.
1) Membership application has been received, reviewed and accepted.
2) Documentation of award of the Purple Heart medal has been received, reviewed and accepted.
3) Payment has been received and Membership number assigned.

Missing or illegible paperwork will result in delays in processing your membership so before you begin, we suggest you have a digital copy of your documentation ready to digitally submit.

You are required to submit ONE copy of any of the following documents indicated below that supports that a Purple Heart medal has been awarded to you. You will be given the opportunity to attach this document to your application during the process and this is the best way to get it to us, however, you may also fax it or mail it to us. You will be given instructions at the end of the application process how to do this.

If Discharged:
DD214 (if required DD215 to show the award of the PH)
If Active Duty:
Record Brief (ERB, ORB or SRB)	General Orders	Permanent Orders	Special Orders
Endorsement Letter	Transmittal of Award Letter or Memo	Personal Awards History	Individual Awards Report

The document must clearly show your name and the award of the Purple Heart Medal. Please note that the Purple Heart Certificate alone is not sufficient evidence of the award of the Purple Heart.

For your date of birth, if it does not already appear on the document, please write the information on it.

Be advised that on request any knowingly fraudulent document sent by you will be released to the Federal Bureau of Investigation (FBI) and may result in prosecution and/or fine/imprisonment.
